Removing the mirror glass from a 2006 4runner

I am guessing you are replacing the side mirror? The glass is glued on to a plastic frame and it does snap out, takes a little umph, but it can be done. The center mirror, will have to be replaced as a unit, don't attempt that one. the side mirrors come off complete with motor and everything by snapping off the black plastic inside and removing the bolts holding it on, They are heavy units, so be ready to not drop it. The glass can be pried out with a flat bar or something. It comes out with holder, and it snaps right back in. Check it out, hope this helps.

Runs rough between 60-70 kms can feel slight

No I would not be real worried. You might want to have your front tires checked for balance. If you get dirt on the inside of the wheel it will cause a vibration in the steering wheel. Also if you throw a weight off of the wheel it will do the same. It could be in the transmission, but usually you will hear a humming sound when you are having transmission problems. The tires being out of balance is usually what a vibration at certain speeds symptoms are.

Check all the tires on your car ( including the spare) once you have checked all the tires and found the one that is low, find the small round black button under the steering column and while the car is running, push and hold the button untill the yellow light goes out.

Toyota 2006 4 Runner: What is the recommeneded

Toyota recommends 5ys or 100k miles which ever comes first. As a professional mechanic, I recommend this at 3yrs or 80K miles for better performance and optimal gas mileage. Also having the spark plugs on the motor 5yrs/100k may lead to plug seizing to the aluminum heads.

Is it necessary to flush the transmission entirely

It is not necessary to flush. If you are just doing drain and fills, I would recomend every 15,000 miles. If you drop the pan and changed the filter, every 30,000. It is not a bad idea to flush on a 60k service and then drain and fill every other time. This does depend on your habits with the vehicle of course. If you regulary tow or a ton of stop and go traffic, this is considered harsh or unusuall conditions and should be tended to more often to maintain system performance.
