1991 F150 automatic 5.0 intermittently stalls in reverse on a cold engine. Recently has struggled accelerating in 1st gear from a complete stop, also sporadic. Have noticed very poor fuel mileage since both issues started. I'm somewhat mechanically savvy & can normally pin-point the issue so I'm down to fuel filter &/or vacuum hoses (since a hot engine would theoretically expand the rubber, sealing any leaks). Please offer opinions.
You might want to check for a busted fuel pressure regulator diaphragm. With the engine running, pull the vacuum line off the regulator. If gas comes out of the regulator, replace it. For the acceleration issue, check the signal on the map sensor. It should change drastically when you open the throttle.

Its possible it could be other things causing the problem, but seeing as you've checked most of the likely causes, it is most likely your Torque-Converter. The lockup clutch in your Stator-Fan located inside the Torque-Converter could be siezed, causing the car to stall.
