1992 Ford Ranger with the 4.0. Extra high idle at cold start-up followed by almost stall extra low idle. Then it chugs for the first 50 ft. Gets better as it warms up.

Ignition component failure. This could be the coil pack, the crank sensor, the ignition module.

If it's surging, make sure your radiator is full. Do a tune up, plugs, wires, etc. Put some fuel injector cleaner in your fuel tank. On a cold start the vehicle compensates for being cold by adding fuel. Once it warms up the computer tells it to quit adding extra fuel. If it is a bad sensor of sorts, check engine light should come on.

There are many cold air intakes available from the after market, also look for a radiator for the 1992 Ranger with an extra core or two rather than adapting from another car.

1. Check and/or Replace the IAC (Idle air control)
2. Check TPS (Throtle Possition Sensor)
3. Check for any other vacuum leaks old rubber hoses are bad to rot and let extra air flow into the intake.

You need to clean the throttle body and idle air bypass valve with spray carb cleaner, hold the idle high while you do this as the engine will try to stall, disconnect battery when done for 5 minutes then reconnect and drive a few days to relearn idle strategy.
