These covers can be clarified ON the car. I recommend Blue Magic headlight lense restorer. It is a cream and applies just like polish, use cotton NO paper towels, or synthetics. You may have to apply more than once to completely remove the pitting as yours seem to be very bad, however do NOT try to do the whole lense at once, the trick is small circles on a small area at a time. Wipe with a COTTON towel/rag to remove the residue, then polish with a clean cotton towel to finish. There are other brands that do the same thing, which I have tried. These products are available at Autozone, Advanced Auto, O'Reillys, and Pep Boys, for about $10-$15. I hope I've answered your question. If there fogged from the inside you'll need new headlamp assemblies. If there fogged on the outside they can be restored. I do headlamp restorations all the time. The trick is to know if the problem is on the inside or outside. Also if the "fogging" is condensation than for sure they will need to be replaced.
