Fail-safe is a limp home mode for the computer to help get the car to a repair shop.
The two most common issues are a cooling system problem or throttle body-fuel pump.
You should be able to have the computer scanned for trouble codes and proceed from there.

SOURCE: ferrari 360 modena f1 speed restriction, safe mode

This great car does not have speed limiter, as you said the limiter is starting because the ECU detects a problem and put the car in limp mode, starting the Rev limiter.

The only way to know the problem is by connecting the car ECU to a code reader, and interpreting codes.

If the car goes in limp mode when you press the brake pedal can be a brakes related problem or even a contact on the brake light wirings, check if the lights are are working.

Ask your mechanic to scan the ECU, this will tell you what the problem is.

SOURCE: Solution for the message "engine fail safe mode" in explorer 2005

I have hearing a lot of this lately with Fords. Most of the time this dash board light comes on due to problems with the Throttle Body (TB)not just your coolant,etc.. If it is sticking, dirty, causing misfires, etc. then this can cause an engine fail safe mode. You might first try putting in some injector cleaner and a higher grade of gas. Many people I know who have had this problem have had to have their TB cleaned and/or replaced. This is something Ford dealerships now about, but hasn't been made a recall or anything. There are service bulletins though.Mainly keeping cleaner in your gas and performing ritual tune-ups/oil changes may be the only thing you can do.Hope this helps

I have a 2004 ford explorer xlt. the indicator says engine fail safe mode. what could be the problem?

Failure Mode Effects Management (FMEM) is an alternate system strategy in the PCM designed to maintain engine operation if one or more sensor inputs fail.
When a sensor input is perceived to be out-of-limits by the PCM, an alternative strategy is initiated. The PCM substitutes a fixed value and continues to monitor the incorrect sensor input. If the suspect sensor operates within limits, the PCM returns to the normal engine operational strategy.
All FMEM sensors display a sequence error message on the scan tool. The message may or may not be followed by Key On Engine Off or Continuous Memory DTCs when attempting Key On Engine Running Self-Test Mode.
With the fail safe mode was the "Service" wrench light lit or check engine light ?

Failsafe engine mode

go to a parts store and have the codes read(most of them will do it for next to nothing. There is also more than likely some maintenance needed. get back to me when you have the codes and we can diagnose this together.
Just because there is a code does not necessarily mean that part is bad, but an area to test to find the actual problem.

Engine Failsafe Mode came on after cleaning battery cables. Vehicle will start, but won't run. No throttle

Fail safe is TPS sensor. When you go in fail safe the vehicle give a set amount of RPM and no throttle response the rule of thumb in fail safe is APP1 and APP2 does not agree the goal is to protect the catalytic convertor. So if the transmission was at fault you dont over rev to end up burning the Cat. Or over rev to break the engine It all boils down to a throttle body assembly as the solution
