I have a 93 k1500 4x4,i have 230k on it, rebuilt trans at 120k, What happens is that i start out 1-2 gear fine, then no 3 rd gear, just revs, but if i manually shift 2nd gear take it up to about 40 mph and shift to drive it goes into drive and runs fine. I also pulled the fuse and manually shifted also, no problem. I have routinely changed fluid and filter, also changed out shifter soloniod, no difference. I also can start with the shifter in drive, take off punching it with the high rpm's it also shifts into drive. Any ideas???

The EMC detected and problem so it's in limp mode, As long as it's in limp mode the 4th and OD will not activate. Take care not to burn up the transmission by running it hard while your in the limp mode.
Your going to have to get the truck scanned to clear any codes and why there are DTC codes. Auto Zone scan for free and most auto shops charge $10.00 to $20.00 to scan sine they want the business.
If you want to clear the codes your self you can put the ignition to the run position and pull the ECM (Batt) fuse out for 20 sec and place it back in then start the truck. It may stall but let it, just start it again and this would have cleared all the DTC code history.
The transmission should be running fine unless there is a problem and the service engine light will be on again and you'll be in limp mode still with an updated DTC in the ECM.
Good luck and hope this helps.

Does it just not shift up to 3rd or when it shifts to 3rd it is just like neutral?

you may be confusing the torque converter clutch engagement with o.d. this trans has a problem with the T.C.C. regulator valve wearing out the valve body bore. this will cause the converter clutch to engage only sometimes or not at all. this will set a P1870 code in the control module. when this code is stored in the controller, ( check engine light may or may not be on), it increases the pressure in the transmission causing the 1-2 shift to be harsh. ask a local tranny shop to scan the vehicle ( most will do this for free) if this is the problem they can either fix your valve body (sonnax parts) or replace it with an upgraded valve body. hope this helps
