I just changed the fuel filter on my 2003 Aviator and it was a pain. Aftwer looking and looking I finally found it. It is located under the truck at the center of the front crossmember behind a shield. Two bolts hold the shield and there are two quick release hoses on the fuel filter itsself. Good luck!!!

Remove the cover in the center of valve covers.
Remove coil packs
Remove spark plugs

It could be a fuel injector problem on cylinder 5. Sounds like a failed injector. Run a compression check on that cylinder to make sure you have at least 100psi. Anything less than that would lead to either a burnt valve or a piston ring issue. Doing a compression check first will save you a lot of money.
