We have a 2004 Cadillac Escalade, 2wd with approx. 52,000 miles. Recently our engine has started making a loud knocking noise and runs a little rough when it's cold. The noise started approx. 2 months ago and it has become louder and lasts longer. The noise usually fades after the car has been running 5-10 minutes (not enough time to take to a mechanic). At the same time the vehicle is knocking, white smoke is emitted from the tail pipe. I've also experienced my heater not working...can this be tied to the issue? After researching online, I've found that it's not unusual for a GM product to have a cold start knock, but the noise has only worsened and I'm afraid we might be dealing with a bigger issue.

Did the vehicle over heat at any time? overheating can colapse the cam shape of the piston causing slap and smoke when cold. it will start doing this when warm after a while. check with dealer as it usually is the same cylinder on all engines. my buick was #8.
