P1522 is not an O2 code in any Mazda model. Please go to this LINK and read about it.

SOURCE: Code: P0133 - O2 Sensor Circuit Slow Response Bank 1 Sensor 1

is this a 4 or 6 cylinder?
if a 4 cylinder it will only have 2 o2 sensors
V6 will have 4 o2 senors which the sensor will be the right side of the engine first sensor.
1st sensor on exhaust, I recommend if you change one change all, as this will make the computer when it rechecks the system you have all clean/fast responding sensors.

if you have a slow response it means the sensor is dirty and not reading well, which is usually caused by minor misfires that the engine computer dismisses, so a tune up would be recommended too.

Bank 1 is the frt side exhaust manifold (did they say bank 1 sensor 1?),you can start by replacing that sensor,i would check the intake for leaks ,the gm 6cly. are known for bad intake gaskets, you will see oil coming out form the frt or rear of the intake, also check your atifreeze overflow bottle for bronwish sludge, which means that the oil is mixing with the antifreeze.A leaking intake could cause that lean condition.

SOURCE: Check Engine Light - O2 Sensor - bank 1 running lean

you should have three oxygen sensors on your car, one for each bank of three cylinders; should be towards the Y pipe on each exhaust manifold. Additionally you should have one on the exhaust pipe before the cadilitic converter.

SOURCE: dtc codes p0155 & p0051

I had the same problem and found out I was sold the wrong sensor, my 4.0 has four, they are all different, when you install a new sensor, you must make sure you clear the trouble code before you start the engine, ignition on engine off, that should do it, it worked for me, also if you have your own scanner and can erase the codes yourself, the wrong 02 sensor will turn engine light on with cold start, then after I erased the code it would stay off all day until the next cold start, thats when I found out my new 02 sensor was for bank 1 sensor 2, and not bank 2 sensor 1, hope this helps

95 Mazda Millenia- P1195 & P1522: I have already replaced the mass air flow sensor all but 2 O2 sensors, and I have replaced the selenoid for the EGR according to the P1195 code problem/solution i

P1195 refers to the map sensor, not the egr solenoid. It could be a bad map sensor, bad wiring, or possibly a bad or loose vacuum hose.
P1522 refers to the air control flap actuator on the intake manifold. You can see about yhat at the link below.

I had a check engine light come on when the vehicle was running bad. The OBD code read P0421. I pulled the plugs cleaned and gapped the, checked wires and started the car. It is running great now, but I...

When the codes were read the first time, did the person doing the scanning then erase the codes? The codes are not erased until the scanner performs that function. Or did the light come back on?
If the code was set again, you will find loads of information here on that code and what it means. Warm Up Catalyst Below Threshold -Bank 1. The front catalytic converter is not showing effective performance as measured by the oxygen sensor on bank 1 (bank 1 is the side where your number 1 cylinder is on). This happened from it running bad. But now great, so...question is how long did it run bad? You may already have a bad converter or oxygen sensor. Pay for diagnosing or try a new o2 sensor, good luck.

it is not an O2 sensor but a heated oxygen sensor that is in the exhaust manifold before the cat converter
heated oxygen sensor(HO2S) 1 bank 1 is the one against the fire wall
it is not a heater as such but has an internal heating system so that it reads the exhaust emissions correctly and adjusts the air /fuel settings in the ECM
normally has 4 wires
the oxygen sensor (O2S) is after the cat converters and is a completely different sensor to read a different exhaust gas

bank 1 sensor 2 is the left downstream sensor (1/2) is located in the exhaust downpipe just after the mini-catalytic convertor, and before the main catalytic convertor. Now the meaning of this is that your not getting any 12+/12- heating on sensor, which means voltage is supplied to heat sensor up. You have a O2 sensor heater relay, but not getting problem with 2/2 sensor tells me you need to check/inspect the wiring for 1/2 sensor, open wiring for 1/2 sensor and inspect
