This could be but not limited to a faulty O2 sensor. Other chances could be a dirty air filter, fuel filter, MAF sensor or even a small vacuum leak from the engine head gasket. If there is a CEL triggered, it would be better to connect the diagnosis tool and check for the codes as this might help you in locating the exact cause of the problem.

have you checked the injectors? might be the problem.

Sounds like you got two wires mixed up. Check your firing order. If not that, then you dropped a spark plug and flattened the gap. If it's a V6 the firing order is 165432. If it's an 8, it's 18726543.

When it runs really rough is there alot of black smoke? Could be the fuel pressure regulator diaphram, real easy to change on that TBI...

Best thing is to get the truck scanned for any DTC, Auto Zone scans for free and can help you pin point the cause of your miss fire. It could be running lean due to dirty injectors, so injector cleaner in the gas tank may help after a few days of running the cleaner through the system. It may just be time for a tune up, spark plugs and wire change ect. Could even be a dirty Air mass flow sensor that could be causing the miss fire.
Have it scanned to save you some time and money and most garage now will scan for around $15.00 now. Always call around for the best price when getting you car or truck scanned, most places have the scanner and would want you business if you decide to have them work on it.
Good luck and hope this helps.
