My 2004 dodge 3500 diesel , when i am driving is reving from 500 rpm to what ever speed im driving rpm range is. it act like its not getting enough fuel .....could it be some bad fuel

Stop up fuel filter or fuel injection pump. pressure low.make sure air filter is not stopped up.diesel engine need to get air in order to run well .check the egr valve and pvc valve .make sure they are not stuck open.
