The Pulse Width Modulated (PWM) cooling fan relay is located below the right headlamp behind the bumper fascia.
Cooling fan relays are usually located under the hood in the engine compartment in the black box with all of the fuses. On the under side of the box cover it will usually give a drawing of the fuses and relays and the position they hold in the box.
Which one ? your van has three cooling fan relays an two fans ! The relays are located in the under hood fuse box # 9, 10 , 12 ! Relay Driven Dual Cooling Fan Diagnostics

Cooling Fan Relay #1, Cooling Fan Relay #2
In the under hood fuse / relay box ! There are three of them COOLFAN/relay 1-2-3 . It's marked on the under side of fuse box lid .
The engine cooling fan system consists of 2 electrical cooling fans and 3 fan relays. The relays are arranged in a series/parallel configuration that allows the powertrain control module (PCM) to operate both fans together at low or high speeds. The cooling fans and fan relays receive battery positive voltage from the underhood fuse block.
The relay for your cooling fan is located Under hood, passenger side, front side of wheel well,
mounted in relay panel. I would also recommend testing or inspecting the coolant temperature sensor as well. it is located Under hood, passenger side, upper engine area, above
water pump, mounted in intake manifold
Under the hood on the drivers side, just forward of the shock tower, you will see what is called an "under hood fuse center". On the cover you can see the various fuses and relays that are located in the fuse center.
One of those relays is the coolant fan relay. Remove the top cover of the fuse center to gain access to the relay.
Hope this answers your question, good luck.
