Those would be po 1684 and po 135 this is your downstream o2 sensor problem.the fix is to have it replaced.thanks for your question.

It is the Upstream Sensor. 100 Codes ending in 30 thru 35 are dealing with the Upstream Sensor. 100 Codes ending in 36 thru 41 are dealing with the Downstream Sensor.

check the vacuum line going to the valve and the transducer, this is the black plastic rounnd thing between the egr valve and the intake, chances are the transducer is bad and can only be purchased along with the valve. if you have access to a vaccum gauge you can connect it to the line at the egr valve and raise the rpm to 2000rpm and see if it pulls vacuum, but you can probably count on buying a new valve assembly.
it will come with the tranducer which is probably allyou need. hope this helps.

depending on what kind of of scanner you used for codes,
cylinder #5,doesn't always mean cylinder #5,
it sometimes means the fifth cylinder in the firing order.
Chrysler's DRB scanner would give us the exact cylinder,
if you connect your scanner again see if you can watch the misfire counts happening,if you can pull a wire of off a different cylinder and see what cylinder the scanner calls it.
