SOURCE: speedometer doesn't work on a 1991 nissan maxima,
If it's like the 92 Maixima it is driven electrically from a sensor on the back of the transmission. It's a round cylinder of couple of inches in
diameter and about 3.5 inches long with a couple of wires attached. Located on the trans above and to the left of the engine oil filter looking from the drivers seat. It could be this sensor or the insturment cluster could be bad.

It does sound like you have gearing trouble in your speedo and that estimate sounds about right. If you're not too concerned with the accuracy of the miles shown on the odometer you may be able to buy a speedo from a junk yard and install it your self or have it installed. If you do the job yourself I recommend getting a manual ( chilton's, motor) to aid you in the specific step by step procedure as these newer cars have seemingly senseless design for securing dashboard components and help you to not break any expensive or hard to find parts.

It sounds like the problem is within the transaxle and although it is fixable, requires the complete removal and disassembly of the transmission to get to the speedo drive.
