Could the drivetrain get damaged from driving on a spare?

Bluntly....... Quite possibly YES.
The skinny little spares are designed to run at low speeds and get you off the highways. If they are a different circumference they will put strain on drive train components.
My recommendation? Time for a visit with your local tire/drivetrain service provider.

Probably find that the spare is loose on the wheel studs or flat
Spare tires are just that spare--- and should be the same as the other tires and pumped up
If the spare is a "skinny" spare , it's purpose is to get you to a tire shop to have the flat repaired or replaced
Legally it is not an every day driver tire
But --NO --it will not damage the drive train.

It sounds like the starter is staying engaged for too long. This will probably result in damage to starter or flywheel if you don't address it. That "horrible nose" is very bad.

Check engine light come on ? You should have some testing done ! Fuel pressure an flow testing , Scan tool data checked . Charging system , battery , connections , cables . What data does scan tool show for mass air flow sensor , what are the fuel trims at ? If you don't know what these are you need to take it to a qualified repair shop .

I had the same problem and I ran the truck down towards empty. Upon filling the tank with fresh high octane gas I drove my truck for 50 miles on the interstate. After that drive my 2006 f150 with a 5.4 ran like new again
