No mention of what motor, but has it been tuned up recently? Any DTCs? Problem can be almost anything - spark plug(s) not firing, injector not firing or leaking, bad compression on one or more cylinders, vacuum leak, etc. If you have no experience invest in having a qualified, reputable mechanic check it out for you. Or you can spend a lot of dollars purchasing plugs, cables, injectors, sensors, etc. and still not be sure that the problem will be resolved.

The fan relays or fan motor itself can be faulty that's why it tends to overheat. You will need to have it replaced if that's the case.

The motor shakes so bad it's because that you have a faulty TPS (throttle position sensor). If the TPS is faulty you'll have erratic idle.

One more reason why the engine shakes when driving is because the transmission fluid is burnt. If the fluid is burnt it tends not to shift so I suggest that you have the fluid replaced.

2002 Dodge Ram 1500 5.9 liter GAS not Diesel, giving code of p1391 which means cam/crank sensor - we have changed crank sensor and it's running worse - we have to start it about 3 times before it will quit...

most likely its the throttle body sludged up . clean the throttle body and butterfly . this will only help if there is an idling problem . ck for codes and ck freeze frame data for more help

thats it may be the fuel pump, and the reason why its worse when cold its becouse, when you start the engine cold it needs more fuel to function properly and hold a good idle until it gets its normal operating temp, during these moment the ecu increases the injectors pulse duration to get more fuel to the engine and perhaps a low fuel pressure will give bad fuel atomization and it gives you bad idle and cruise quality, I recomend you to check your fuel pressure for abnormal readings it can be easy by the injectors fuel rail, i hope it help's good luck
