The shifter cable may have a cracked cover at the bracket and it allows the shift lever on the transmission to move to neutral during the stop and when the car levels itself now the lever goes back into position and the transmission slams into gear because you are trying to move applying gas peddle also check the transmission mount for broken disconnected rubber that holds the transmission to the crossmember the transmission is moving and that's what broke the insulation on the cable at the bracket
The Vacuum Modulator sounds like a good place to start. I would
change the Transmission filter and gasket along with the Transmission oil. Dirty Transmission fluid with a dirty filter can clog
the valve body and cause this.
Sounds like an accumulator seal is leaking or a clutch pack seal. Checking fluid level is a good idea and a place to start. Check for external leaks also.
You need to get the shift synchronizers checked out could be that the clutch needs to be adjusted
Ford transmissions had an aluminum piston on one of its clutch packs that cracked letting fluid leak out. At idle not enough fluid is being supplied to keep the clutch engaged - especially noticeable when hot. Ford made a stainless steel replacement.
If its automatic check the fluid. Sounds like you might need a transmission filter. The bands or gears are dirty.
The Ford AODE ( automatic overdrive electronic ) does not have a modulator valve. more than likely you have a shift solenoid going bad. This may or may not set of the check engine light until it permanently fails. If your check engine light is on the first order of business is to make repairs to turn it off.
Had to to happen on my 94 Cougar, had to replace the manual gear position sensor.
There is a part on the side called a "vacuum modulator". It seldom goes bad but sometimes the vacuum line going to it will get dry rot and leak. Check it out. First check to see if your tranny fluid is not burnt out. That stuff does go bad with a lot of mileage.
First you have to make scanner for your transmission.
Second check oil level.
Third check your transmission cooler
Sounds like a possible misalignment problem . . . have you hit or run over anything that could have hit the engine or transmission really hard ?
The other thing it could be is a broken Motor Mount which will misalign the tranny too.
Hi ure problem is the sinkrow the not machnas that courses it to slip out as u are to get it seen to soon as possible

Dodge has a safety feature on there trannys. Its called limp home mode. If the computer detects a problem with the transmission, it will eather put it in 1st or 2nd and not change just to let you get out of the road or home if your near. But when you restart the car i will reset till it detects the problem again. But for as what the problem is it will be hard to say. With computer trannys you can have it hooked up and scanned for error code.

-Check transmission fluid at idle in park position. If low, add more to full level and try to drive. You should be good to go.
-But if fluid is full. It's time to rebuild or new transmission
-When check fluid the engine must running and hot.
Good Luck

Agree filter, fluid chng, check vacumn for leaky hose, or deterioration
Check your fluid. Your fluid may be low by 2 quarts. filter change is maintenance and a good thing to do but won't help your problem. a bad modulator hose will just cause a late shift and is not the issue here.
Check fluid
Hi I think checking fluid level would be the best solution and if your fluid is fine please try to give it a service (maintenance) which is replacing a filter and fluid may be the solution to that.
Start with fluid check, then modulator, and after that see about adjusting bands if applicable
Check the OD lamp in dash or gearshift lever. If On or flashes every time starts engine, transmission fluid is not good. Replace filter and if possible all fluid (Check user manual for type of fluid) and drive it for a 50 miles continuously. If light turns off just add a transmission fluid conditioner (available in auto store car parts). Check drivetrain every 10,000 miles.
