Could be a number of reasons, fluid level is one,plugged filters, fuel& tranny,what does the transmission fluid look and smell like , they are good indicators as to tranny issues

you need to replace clutch pack for that gear can be ordered seperately from parts store or dealer if you want to save money call peach street auto recycling at 1800-365-2341 for a warrantied used one! also might be time for change trans filter and fluid filter could be slightly clogged but I lean towards clutch pack I replace these alot on Hondas

check the kickdown/throttle valve cable. It may be bound up and stuck in the maximum position giving you extremely late shifts.

I have the same truck and I had the same issue where one day I was heading to the dump and my truck wouldn't shift out of second gear. So I had to head home and I took the tranny cover off and checked it out and nothing looked bad but what I found out was there are 5 shift silinods and I changed 1&2,3&4 and it worked.so before you go replacing your tranny try those first.
