chances are the power steering pump has seen better days and may need replacement. i dont recomend any additives in these systems. if its making noise, there isnt really alot you can do. dont replace the gear box unless there are leaks.

It could be a voltage regulator problem. or even a alternator problem when the volts drop the car looses electrical current causing the rough idle. try replacing alternator.

I recommend that you take it to your local Mercedes mechanic to run an engine diagnostic test. It's not likely that you local auto parts store will be able to help you. You don't necessarily need to go to the dealer, since they normally charge the highest prices; however, my local dealership charges $125 for and engine diagnostic. I don't know what you consider expensive.
