Mcdevito75 here, It could be a few things, Fuel filter, fuel pump, the need for a tune up, one of the engine control sensors going bad. Have your local shop run diagnostics.

Car running perfectly. Accelerated to 40mph, cruised in 5th gear till approaching exit ramp, downshifted to 4th, let up on clutch pedal, then no acceleration as if clutch was still disengaged.

either your clutch has failed or the input shaft on the trans failed, either way it will be necessary to remove the trans to determine the failure

04 saturn L300 acceleration dies

run the fault codes first to check for faulty sensors
what you describe indicates a problem in the exhaust in that when the exhaust gas volume reaches a certain level the sensors are reporting it and the ECM is not able to fix it
have an accredited exhaust shop check the cat converter
for failure and partially blocking the exhaust at speed
if it was a fuel pump you would run into problems long before the speed at which it occurs
AT best it could be an EVAP problem with the vent not working properly but I think that it is exhaust related

Engine light is on- gear shifting problem

The codes that you listed are all transmission shift control codes. You need to have it checked by a good transmission shop.

My car is jerking while accelerating and reaching certain RPM

I would look for a fault in the ignition system such as a cracked spark plug. Might need a scope check to put your finger on the problem

Jetta will not accelerate up inclines.

good chance you have a clogged cat converter ...with engine running have someone rev engine and see how much flow you have out tail pipe. also if you hear the sound of something rattling inside exhaust that is another tell tale sign
