After you fail, you go to a repair shop &
resolve ALL Vehicle issues
Then the shop road tests until all 8 I/M Monitors
run clear ,or there is no reason to go back to inspection
knowing your going to fail.
The problem is the shops & peoples inability to explain
what they need taken care of.
If you never work on the vehicle for years,then you don't
go to inspection,it is a given you will fail
Spark Plugs-every 3 years
Check for Cylinder Misfires
Always install a New Air Filter before inspection
Always install new fresh full sys oil before inspection
Oxygen Senors get replaced every 100,000 miles
Have to road test with professional scan tool & look at
all data,check monitors,fuel trim,O2 etc & check for codes
You have to spend the money to evaluate the vehicle, to see
if it is ready to go for inspection,not many are,their just driven
& neglected
Rich could be-- O2 Sensors,Dirty MAF Sensor,Vacuum Leaks,
Injector Leaks & more,I have seen a dirty air filter fail a car
Could be lots of things. It could have a leaky injector that is dripping all the time. You can pull the injectors while still on the fuel lines, to see. You could have bad compression and burning oil. You need to do a compression test to tell. You could have a bad plug or wire. They are cheap to so just change them. The fuel injection system could be off. Check the temp sensor, air mass sensor, O2 sensor, with an ohmmeter. Don't remember the values, but bad ones usually go 0 or infinite.
Your engine is still running rich, inspect all your vacuum lines and your fuel pressure regulator. A bad fuel pressure regulator can cause your engine to run rich and is there any check engine light on???

I've seen two cases recently where it was bad distributors causing rough idle. An honorable muffler shop (not a big chain) will test your catalytic converter for free. Is a '91 throttle body? Do Fords have cold start valve or injector? This can cause rich reading if malfunctioning. Anything controlling fuel delivery into engine can malfunction sending extra fuel. When you find problem; do you know clean oil will help pass smog test?
check for vacuum leaks especially around the intake manifold gasket. Although the engine appears to be running rich, it could also be misfiring due to being too lean in one cylinder. When the cylinder doesn't fire the charge, it goes into the exhaust completely unburned and looks like a rich running condition due to the unburned fuel. If it is a vacuum leak causing the problem, the engine will run fine at road speeds but misfire at idle and not pass smog.
