Why does the phone need to be Flashed? That could be the reason the Nokia software isn't reading the phone as being there.
Can you get into you contact list on the phone itself? You could do it the old fashioned way and write each of the numbers down with a pen and paper. You can also go into the contacts section on the phone and move the contact information onto the Sim Card from the phone's memory. That way when you remove the Sim Card the contacts go with it. Unfortunately you will probably have to move these numbers one at a time.

There are very few reasons why a phone asks for a PUK code. The most likely is that the phone is stolen, or reported stolen, so the phone provider blocks it from being used. The second most likely, it the phone does actually belong to you is that someone has tried to open it 4 times using the wrong code. And the third most likely is that you have put a SIM Card into it that is different to the company that owns the phone under an undischarged 2 year contract.
