Uninstall the APP on the iPad and then reinstall it. If this doesn't work, I would suggest restoring the iPad. Connect it to iTunes and restore it. Or you could do it from he iPad itself.

If this is an App, just put you finger on the app icon and leave it there until all the app icons wiggle. Lift your finger off and press the x in the top right corner, and then confirm the delete. Press the Home button to cancel the app wiggle.

Your iPad is in restore mode. If you boot the device while holding the home button, it will boot into restore mode for a clean firmware install or a restore from backup.

you can either hard reset the device by holding the sleep/wake and home buttons down simultaneously for 8 seconds, tether it to a computer and complete the restore, or just leave it alone for about 20 minutes and the restore timeout wll kick it back into normal operation as long as there is a good firmware install present

Get your daughter to go into her Facebook account.
Ask her to go into account settings. You will find an area that allows you to sign in it of ALL devices.

This should disconnect her Facebook account from your iPad.
Uninstall the Facebook app. Soft reset the device (power and home button) then reinstall the app when it comes back on.

The information included with your question says that you have a 3G iPad, but it does not indicate that you have WiFi. If you have WiFi, touch the Settings app and, if WiFi appears, make sure that you are logged into a local WiFi network. If you have a 3G-only iPad, you must arrange for 3G service with your current, or another, phone provider. 3G service is considerably slower, and more expensive, than WiFi.

Try this:
If you can't remember them your codes; then go to Express Lane and select 'iTunes' from the list of 'products' in the middle of the screen.
Then select 'iTunes Store', and on the next screen select 'Account Management'
Next choose 'iTunes Store Account Security' and fill in that you'd like your security questions/answers reset.
You should get an email reply within about 24 hours (and check your Spam folder as well as your Inbox) - but some recent posts are saying that Apple have temporarily suspended the reseting of security question/answers whilst account security processes are improved, so it may take longer.

I hope this helps. I use Outlook notes to keep track of my passwords; I have SO many!
