When I got my Paperwhite I had the option to set up a password or not; I did not. This kindle should be dirrerent from past kindles you may have had. Try giving it a new password. If that doesn't work; contact customer service and they can clear the forgotten password and you can start over again. You can still go through the rest of the set up while you wait for them to call back.

Contact Amazon customer service. They would be the only ones that could find out where they went or why they can't be accessed.

This is an Amazon problem. You need to call customer service and ask them why you are not getting the refund. Keep going up the chain of command until you find someone that can tell you why you aren't getting it.

First off, I've had 5+ of that type of kindle. Each time the screen was cracked causing the screen to not turn on. If you see any lines this is definitely your problem. If so, you can either replace the screen or replace the kindle.

You have asked about the Kindle eReader, but your question refers to a Kindle FIRE. On the eReader touch the Aa button and select the correct orientation that you require. On the FIRE drag down from the top to reveal the top MENU - select the CIRCLE icon on the left which is the SCREEN LOCK icon. Turn it off.

You are SHARING your Amazon account so that it currently sees your 2 Kindle devices and his Fire. This is great if you want to share books and stuff. However, it does mean that it will only relate to YOUR Amazon account details i.e. YOUR email address and therefore YOUR Facebook, Twitter and all the rest. If he wants his own setup then he will have to de-register his fire from YOUR Amazon account and register it to his Amazon account. It's that simple.

You posted this question for three different Kindle units. Since two of them were for the Fire, I'm assuming that's what you have. If you have something different, please specify. For the Kindle Fire, press and hold the power button for fifteen seconds or so, ignoring the "Shut down?" prompt, until it turns off.
