The alt/az adjustment knobs don't work
Are you sure the set screw is tightened? Some scopes have limited adjustment ande the screw need to be "centered" so you can adjust in both directions.

Unable to focus with magnification greater than 40x
You must collimate the scope. Watch this video.

Also the maximum magnification of any telescope is NEVER more than 50 times aperture, so your scope is 400 power. BUT everything including the sky must be perfect. Usually 30-40 times aperture is the most you will ever get.

They can be replaced. Call a local camera repair shop and ask. Find a
local Orion dealer and ask what they would charge (not all of them can service telescopes). Get in touch with a local star gazing club, often members will know of places or do it themselves.

Can focus near not far
Try using the eyepiece with the largest number written on it -- do not use the 2x barlow... just the eyepiece.

Telescope tool, like a swiss army
For what purpose? Screwdrivers and maybe an adjustable wrench are the only tools needed. You will need an Allen wrench for collimation if this is a reflector style scope.

Most of these small telescopes do mot have manuals--
However Meade maintains a web site for all of it's telescopes that have manuals-- look around the Meade manual web site and find one that is similar to your scope-- and use it to learn about that style of telescope-- manuals are free for download.
