Hi, the PUK code is the security code for the sim card and not the phone, now when the sim card was new you would have been given the unique PUK code written on the card the sim card was attatched to.
if you no longer have this card then the only other place you can get this code from is your service provider, all you need to do is call them and give them either the phone number of the sim card or give them the number written on the back of the sim card, they will give you this code over the phone for free.
the other option you have open to you is if you dont need to use the sim card thats asking for the PUK code then you can go out and get another sim card and this will come with its own unique PUK code.
please dont try guessing the PUK code as it can be up to 11 digits long and they are totally random so guessing is beyond the question

the personal unblock code puk is provided by the network provider , contact your customer care . dont enter wrong puk for more than 8 times your sim will get permanently damaged.
Do not enter any code into your phone. The PUK code is a code that your
service provider has and not one that you would know or have in your
manual. If your an AT&T customer they do provide PUK on their
website. If you have entered in a code and the phone is now saying PUK
blocked you'll have to go to store to purchase a new SIM card, you may
want to call your service provider as well, they may be able to give you
a credit for the new card. Hope this helped.

Your service provider can give you the PUK code. Once that is entered you will be able to reset the PIN code. DO NOT try and guess the PUK code! If the PUK is entered too many times incorrectly it will deactivate the SIM permanently and you will have to purchase a new SIM.
