For PUK ..you have to contact your network operator ....

First off, open Nokia PC Suite. Then click connect a phone. then plug in your phone and put it on nokia mode it might work. And then hit the button in the top right corner on NSS. I have tried to change my firmware code so i can upgrade it but it didnt work for me good luck.

you have to call (using other cellphone but the same network) to ur network's costumer service and tell them what happen and they will ask you some questions... base on my own experience it takes 7days before they tell you the code,..
