What has happened is the hex part of the bit has twisted slightly making it hard to remove. What I would do if you are comfortable with partial disassembly is remove the quick release mechanish. There is a split ring that comes off first and then a washer and spring, next remove the outer collar and SMALL steel ball from side of chuck. I take a pick (dental tool) and push up or try and twist the stuck bit. Sometimes it takes while but usually this works. Let me know if I can try and help further.
I've had the same issue and resolved it with 2 part epoxy. Clean what's left of the broken bit still stuck in the chuck so it's free of oil/grease/dirt with some brake cleaner or similar. Mix epoxy (an eraser's worth) and apply to the tip of a ground off nail or similar and allow epoxy to set. Now you have something to hang on to. Release the spring loaded chuck and pull on the nail - broken bit comes out. Worked for me.
My friend just had a problem where the shaft of a socket adapter broke off inside the the driver. Taking the spring assembly out didn't help as it was wedged so he used a screw/bolt extractor (ez out style thing). Drill a 5/32 hole in the broken piece, pound in the extractor and wiggle it back and forth to get it out. Beats buying a new chuck, or whatever that end piece is called on these drivers...
I had this problem with my makita impact driver. I tried all the online suggestions but nothing would work to remove the small screwdriver bit. I couldn't get anything to grip it. The solution ended up being using a pair of small wire angle snips - they easily gripped one side of the bit so I could slide the collar forward and wiggle it out. Hope this helps anyone else who comes across this very annoying problem.
I had a more serious problem. I think I used a bit that was not compatible with the chuck and it got really wedged in there. I tried clamping the bit in a vice and wiggling, it out to no avail. I then tried a little reverse impact while clamped in the vice, and snap, the bit broke off inside. It was hardened steel, so no drilling it out. The bit was so jammed, I had to replace the anvil. The following site had a great video on how to get into my drill, and had the part in stock too. Brilliant. thanks for all the posts.

Ive heard the if you place the driver bit in a bench vice it can be wigged out. It work for a work mate.
Thanks for your suggestions but unfortunately didn't have much success. Finally broke down and took it into a factory authorized service center. They had to disassemble the impact driver and replace the "receiver" to the tune of $85. I suppose I could have fixed it, but did'nt have the time or attention span. High quality Irwin #2 phillips is stuck in a Makita BTD142 impact driver quick release chuck. It seems like the release ring is broken. I can pull the release ring out but the bit won't move. I've gripped the bit with vice grips, no help. Anyone have any ideas? I bought in January but I can't find the receipt for a warranty repair.

I have the same impact driver and unfortunately the same problem. I've tried to extract the phillips bit with needle nose pliers but cannot get the chuck to release....... Any ideas?
