You need to call your service provider for the puk2 code (warning--do not attempt to enter your puk or puk2 code because the more you enter incorrect codes, the more your phone will get screwed up) the service provider should ask for your information on the person it was registered to (they will ask for the last four digits of that persons social security number.) they will tell you the code and you will have to enter it into the phone (i would write this code down just in case you lock it up again).
