A request for a PUK code ("Personal Unlocking Key") will appear on your phone if your SIM card PIN code is entered 3 consecutive times incorrectly.

Your SIM card will then become "PUKed" or Blocked. This is a security measure to protect your service.

You need to call the Customer Service department of your cell phone provider and they will give you the PUK code which will unblock your SIM card.

I have an lg prada ke850 touch screen phone and I was trying to set in a security code but instead I just locked my phone. Can you please help? I don't know how to unlock it!!!!!
This is a small tutorial on how to hard reset the lg prada on request for forgotten security code. Turn the prada on without a simcard and wait for the insert sim screen. On the touch screen hit call, after enter this code 2945#*#. and you will see a menu pop up headed "Engineering menu". scroll down to factory reset hit that and wait for it to reboot.
