it should be ether in the manual or somewhere on the boxing, it will be a big number and will look like this for example, 123-456-789-0123, if not you should contact EA and ask them for the code.

Unfortunately there is little you can do legally. Unless you find the manual or perhaps contact the develepors help line and see if they can do anything you're pretty stuck.

It is probably be a faulty LCD crystal, or a bad internal connection where the electronics meet the LCD. I would think it could happen if the monitor is jarred or dropped, I guess it could occur also if the monitor was brought in from the cold weather and condensation occurred inside?

Unless you have access to a trained LCD technician, your only option is return the monitor for exchange or repair, or buy a new one.
