These are known for the spring in the nose (#12) to fail.
Using an o-ring pick or small screwdriver remove the hog ring on the end of the bit holder (#17).
Slide the washer, spring, and sleeve off (Be careful not to lose the small lock ball)
The spring that pushes the lock ball forward (#12) is usually the culprit.

Open the tool and control the purple and brown colored wire where the slide contacts are fitted on the carbonbrush holder or the switch itself.

Remove the two sliding contacts loose and squeeze the contacts slide a bit and then connect again. If unsuccessful buy a new switch as well as battery connectors are checked.
It only accepts 1/4" hex shank bits and nut drivers. Mine just pops in and to release it, you push the black collar around the bit down. Cannot put round end bits in it. Only hex shanks
First are you using the correct SDS style bit? They do not use just standard drill bits. They have two long cutouts on each side and two oblong ones on the opposite side. You should be able to push (and rotate to line up the slots in the bit with the tool) the bit in without touching the bit holder and it should lock into place. You push down on the holder only to release the bit. The holder is spring loaded with a small metal ball that contacts and holds the bit at one of the oblong sections.
Keyless Chucks Open chuck jaws by turning plastic collar with fingers and insert shank of bit about 3/4" into chuck. Tighten plastic collar CLOCKWISE while depressing spindle lock button (F) on the right side of the tool housing (Fig. 4). To release bit, turn plastic collar COUNTERCLOCKWISE while depressing the spindle lock button (Fig. 4). NOTE: DO NOT DEPRESS LOCK BUTTON WHILE OPERATING DRILL or while the chuck is moving.

DW660 is suppose to be a tool free bit insertion and removal. However a wrench may be necessary. If you have loosened the the collet nut either with the collet grip or a wrench and the bit does not come out. Remove the collet nut completely from the tool. Remove the the collet with the bit in it. With a couple of plyers, one use to hold the broken bit the other to twist the collet. Sometimes when the bit is overtighten the collet locks on to the bit so to speak and the collet wont expand back out thus releasing the bit by doing the above it will expand back out and release the bit. Reinstall the collet and collet nut.

Are you using a SDS bit in this tool? If so you might have a problem with the bit lock mechanism, if not you do need the special SDS bits that will lock into this tool. Let me know
grasp the chuck and put drill in reverse position and pull trigger gently this will open chuck and re-insert bit do same in forward to tighten bit into chuck
