Your SIM code was locked because you gave it 3 times incorrectly and now phone asks for the PUK code to unlock the SIM.
PUK stands for 'PIN Unlock Key'. It's a unique code that is linked to your specific SIM card and is usually 8 digits long.
The PUK code is unique to your SIM card, so you can only get this from your mobile network. Sometimes they will give you this when you first get the SIM card, but not all networks do this. If you cannot find it, give your mobile network a call or visit their Cutomer's Service and they will be able to give you the PUK code after you have answered some security questions.

Call the Idea Customer Care. They will give you the Puk code after asking your few questions related to the Sim Card.
Details about Puk Code:
This problem is very common and most users think that it's related to phone, which is not true, this is only with your sim card and is for security reason. We can't use software or hardware or other tricks or methods to unlock it, the only solution is from the Service Operator/Network Operator.
If you keep entering random codes/unknown numbers several times, your sim card will be rejected and will not be usable anymore.
Your only solution is to call the service provider (Network operator's customer care), ask them to give you your puk code, they'll promptly give it to you after few inquiries to make sure you are the true owner of the sim card. (there's no charge as well, and if you call from the same network' sim card to the customer care, you'll not spend/pay a penny).
There's no other alternatives, or no other options that you can get the code, so don't play around, rather make a call to the customer care of the sim card network operator and get the code.
The Puk code is a special number that identifies your phone and the cell phone manufacturer and your cell phone carrier can give you the code. Go to your phone support page on your carrier's website, or type in puk code inthe search bar on their website you can also call their customer service phone number and they will tell you the code.
Your PUK code can only be obtained from your network providers, and yeah they give it you for free! Call them up and ask them for your PUK code, or have a look on their website for a link on where they can give it to you online. It should be under the Help section...
You should have a PUK number, on the plastic card where your sim card was.
Only your network service provider can give you that, if you lost the original.
