You will more than likely need a 1/4-20 screw/bolt, or a 7mm. You can find these at any Lowe's store. They are in the "specialty" drawer section of hardware in the N drawers.
Technically no. You can buy just a new mattress, but some of the damage done to mattresses ie the indentations are caused by insufficient frame support. So check your frame to make sure it's in good condition with nothing broken.
