Aside from the other solutions i did removed the dispenser tray and cleaned it with spray bleach and a brush as well as inside the dispenser cavity in the wash machine and 1/2 a q-tip in the holes above. not much change. then i filled the fabric softener holder with bleach on hot, quick cycle twice, and it works! i plan to do this periodically as well as keeping both the door and tray open after each cycle.
Fabric softener is to slow mix with rinse water in the rinse cycle... check the cold water supply line for a clogged screen or a clogged dispenser box cover... hope it helps

Remove the dispenser tray. Inspect the area just above the fabric softener portion of the dispenser. There should be a series of holes where water feeds into the tray during the rinse cycle. Sometimes these holes become clogged with fabric softener residue. If the holes are clogged, water will never dispense the fabric softener out and it will stay in the tray. If you take some cotton swabs and meticulously clean all the holes out, I bet your problem will go away. Let me know if this was helpful to you.

Is the softener entering the machine; that is at the appropriate time? You can take a peep at this time, if possible. If it is water it is okay; this is just the remaining water. The softener is dissolved with water and flushed into the machine.

You need to poke all the holes clear in the top of the dispenser housing with a piece of wire or similar.
This should solve the problem.
