when a phone displays Sim lock, it means the Sim card is block and it can only be unblocked by the service provider of the network you are using. When you put wrong code, the third time it gets blocked and displays PUK code.
PUK code can be given only by your service provider which is specific for each and every mobile number.
Contact your sim customer care department over phone.

If you cant contact through your phone get some other phone and contact them.
