The PUK code or Personal Unblocking Keys are a security device that deters people from using your handset. For as you see 3 failed attempts at your pin means you need a PUK.
The only place you can get a PUK is from your service provider.
This means that you are going to have to phone them up from another phone.
You will be asked a series of questions to verify you are in fact the true owner of the handset and you will also be asked for the handsets IMEI number. This is located on a sticker underneath the battery or you can get it by typing *#06# into the blocked handset.
As I say...Only your service provider can issue you with the correct PUK and guessing will ultimately BLOCK your handset permanently.
