Incline motor is bad or needs calibrated, try this
1. remove safety key
2. press and hold the speed+ and stop keys at the same time
3. Put the safety key in/on and release the keys
4. press stop once
5. hit the incline + arrow and let the incline go the whole way up and down
6. remove safety key and try
The problem is very simple and despite alot of words, the fix is easy too. But before you do anything, unplug the unit and continue reading what the problem most likely is (at least this was the problem with mine and it is back to normal). At the very top of the threaded incline bolt that screws itself into the incline mechanism there is supposed to be a screw and a thick washer attached to the end. Over the course of time this small screw works it's way out (most likely stripping the threads of the small screw itself) and the washer that is supposed to stop the incline screw once it is fully inclined (the resistance the washer creates toward the bottom helps the incline censor calibrate itself so it knows when it is in the full incline vs. 0%. Take the cover off the front where the incline raises. There are three screws in the front, remove them. Now tip the unit on it's side. There are two more screws you need to take out to get the cover off completely closer to the treadmill belt.. Now with the cover off you have access to the incline motor and the long threaded incline bolt that is attached with cotter pins through the clevis pins. You need to take the cotter pins out and remove the two pins so that the incline bolt and the incline unit are attached only by the wires. Don't disconnect the wires! Unscrew the incline bolt completely out and see if the screw and washer mentioned above are still attached. If not, look into the incline mechanism to see if the screw and washer are there. If they are not, look around inside the housing to see if you can locate them. Once you find them, simply put the small screw and the washer back into threads of the incline screw. I recommend using locktite on the threads to prevent it from working its way out again. I had to find a comparable length screw to replace the small one because the threads were so damaged and the screw was actually bent from being wedged so many times inside the incline mechanism. Now before you put the small screw back in, screw the incline screw back into the incline mechanism as far as you can. Now put the small screw and the washer back in with the incline screw back in the unit. Put the pins back in and secure them with the cotter pins. You may need to unthread the incline screw to get the bottom pin back into alignment with the holes. This is okay because the treadmill will recalibrate itself once you turn it back on. take this change to clean everything in and around the unit, hair, dust, dirt old sock! Put the cover back on, place the screws back in the front, plug her in and put the key in. You should be as good as new. It will automatically raise to the highest incline, don't worry, it will come back down to 0 incline once it can determine where that washer you just put back in hits the bottom of the incline mechanism. Hopefully this relatively quick and easy fix helps someone else. It's really simple when you study the moving parts for a few minutes and visualize what purpose this small screw and washer plays. Do a Calibration on the treadmill. Take out the safety key. With 2 fingers, hold down the stop button and the speed up or spped plus button. keep holding them down as you insert the safety key, then let go of both.
Press the stop button 1 time, you should see all 0's in the windows, or EP 2P or something weird like that.
Press the Incline up button and then wait. If the incline go up until it stalls then down until it stalls the stops at the bottom, the you are recalibrated, take out the safery key and put it back in to see if all is well. If the incline takes off up again without touching a button, the you have wiring problems or control problems. Call service company to get it repaired.

Check the motor and the gears that run the unit up and down. If is located on the front of the unit under the face plate. make sure everything is power everything. Also check the gears to make sure they did not break and check the tracks to make sure it did not fall off the track causing it to stay up

I had the exact same problem.
This usually happens if you switch it off at the mains while the incline is adjusting or is somewhere other than lowest. You should always make sure before finishing off, to lower everything.
Now for the solution: If you open up the case it has 5 or 6 black philips (cross head) screws (use your user manual if you don't have your original go to proform and find the support link and download the .PDF if your model is not available get the one before or after in the same model product line (model number is below the power input)).
Make sure you remove the power before opening the cover, obviously.
Anyway, once opened, you should find a sheet of paper wrapped around in some plastic wallet. On that paper it should explain everything. Try not to mess with too much things in the settings, otherwise you might mess it up badly.
Mine is something like: Hold the speed up (+) button and then while holding the button insert the key - that will get you to the service menu (obviously the power has to be connected).
Once in the menu pressing the incline up does a self-calibration (maximum up and maximum down) after that it should be perfect.
Remove the key any time to come out of that mode and resume normal operation.
I may not find this place for any more replies 'cos i only found this site by accident. While looking for info. on my burnt out motor model: PETL 3013.6 .... But i have come across someone selling a faulty one on ebay for like 5 quids... it's picking it up the trouble!

ICON CALIBRATION INSTRUCTIONS- INCLINE
1) Press the Stop and the Speed Up keys down while inserting the safety key.
2) Press the Stop key one time. The Time window should read EP:2P. Some newer models may also say FP, Pass, or the like. This means that it passes the EPROM test.
3) Press either incline key and the incline will calibrate automatically.
4) THIS IS A VERY IMPORTANT STEP BECAUSE YOU WILL GET AN ERROR MESSAGE IF THIS STEP IS SKIPPED.After completing calibration of incline, press the Stop key and then remove the safety key.
If the incline moves but you it will not operate normally nor calibrate, check the wiring from the incline sensor to the power board. If all looks normal, put it back in calibration and see if you see a blinking “- -” symbol in the incline window when you are calibrating. If not, the incline sensor is bad.
If the incline moves but only just a bit, it could be a problem as simple as the gearbox on the motor being jammed. We remove the motor from the treadmill, reconnect the wiring, and then try to run the incline motor outside the treadmill. Many times, this will unjam it if that is the problem and then reinstalling it in the treadmill will get it to work. If you have the same problem or it doesn’t run outside the machine and you can hear it try to work, you need to replace the incline motor.
If the incline motor does not move, we need to figure out if the motor is getting voltage. Use a simple multi-meter and set it to AC volts. When you put it into calibration, it tries to calibrate the directions one at a time. Measure across the white and black wires first, then measure across the white and red wires second. If the motor is getting voltage you will read 120VAC (or
thereabouts) one one of the measurements. If you have voltage but the motor won't move, you need to replace the motor.

If the incline motor does not move and you have done the voltage test and you do not have any voltage present, there is a problem in the electronics board that supplies the power. If the console is powering the drive motor as normal, you can bet it is getting an incline signal and the power board needs to be replaced (some models have an integrated power board/controller) so if there is only a single board the whole board has to be replaced. It is possible the console is not giving the proper signal but we've never seen it happen in thousands of treadmills.
