The PUK code is dependent on your sim card that you have installed. therefore if you have 'locked' your mobile you have in effect locked access to your sim card. you need to contact your sim card supplier.
only your service provider can give you a puk code (orange, t-mobile etc.....), just call their customer service tell them your problem, give them your imei number (normally found under the battery) and they will give you a puk code. Hassle free, this is a common problem.
The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.
