This is one of the tricks to lure you into Facebook. You also are blocked from certain levels, if you don't ask friends on Facebook to like you and the game.
I did trow the app from my phone after lever 20 or so, because of the same annoying things. Yoou can't fight it. Become a Facebook user, or stop playing candy crush.

You can just opt out by clicking the 'x' at the top (the one you mentioned). Let your GF know that this is feature of facebook that allows a user to suggest friends to his/her new friends.

You need to go in to account settings and make sure that your photos are set for friends being able to view them and not to 'only me'.

For sure your friend change his or her profile name, the solution:

1. Ask him in person so that you will know his or her present name in facebook;
2. Contact him or her by phone or cellphone if you have number; or
3. Ask your other friends that you think were also friends of the friend that you're looking for.
