There is no factory settings for windows.
You can restore the system to it`s old position from "system recovery"

First, give the screen a good cleaning using a microfiber cloth. This may inadvertently remove the lines even though they are in the display.
If this doesn't work, you'll have to send the laptop back for repairs. Screens are not a component that can be replaced easily (such as RAM or a hard drive).
If your laptop is not under warranty, scope out some local computer shops that can replace the display for you. Make sure to ask them for a quote BEFORE they do work on your computer, otherwise they have the right to deny you your laptop, once they fix it, until you pay up (a possibly very nasty price).
How to use the pen
Using the pen is quite simple.
You extract it from its storage space and use it as a mouse, left clicking it as you would a mouse.
By pressing the tip of the pen on an area on desktop or on an application window and maintaining the pressure for a short while, you activate the context menus (the equivalent of a right click.)
On the Windows task bar (for Windows XP) there is a small icon near the start button that shows a pen pointing outside: that is the input panel. Click the icon with a mouse or your pen, and an input area opens. In that area you write with the pen (writing Pad) or enter letters and symbols one at a time (Character Pad) or use a virtual keyboard.
Under the red X that closes the input panel there is a small gear. C;ick the gear with the pen or a mouse to configure the settings.
Your pen may have an eraser at the rear end and/OR on the side. It is used to erase characters or whole sentences.
To actually record (write) what you enter with the pen you must have a text-editing application open and running.
Whenever you approach the pen to the screen near a text-area (search area of IE or other compatible applications) a small icon appears to be clicked to enter text. This happens is you select UNDOCK in the input panel configuration.
This should get you started with the pen. Try these steps, and if you have any other questions post them as comments to this thread and it will be my pleasure to answer them.

try to boot to windows xp or dos command, for winXP go to recovery console-select your windows-type the admin password then if C:\WINDOWS come up type chkdsk /r if that doesnt help, repair your operating system or backup your files and reinstall your operating system.

OK you simply need to install the right driver. Identify your Modem and find the driver install it and you will be OK. The driver should be on the disk that came with your modem. Otherwise you will have to use another machine to download.
Howto reset cmos on toshiba 3500
You can just use a paper clip (straighten to a U shape) and short out the battery contacts.
If you are not sure, just leave the battery out for 20+ mins then put the battery back in the holder.
The BIOS settings will then be set back to th default settings, then go into the BIOS setting and set the Date and Time etc.

This laptop can support up to 1 GB of PC13 type of RAM, this unit usually came with only 256 MB of RAM so you have an option to put the maximum amount of RAM for it which is 1 GB.
My Portege M400 will not turn on with the battery
You look very much the unlucky owner of a busted battery.
I'm afraid the battery will have to be replaced. Meanwhile, do not use it with the power supply; not only it will do no good, as you already determined, but it might damage the power supply itself.
